All the fields marked with *, in this form are mandatory and must be filled for processing an application.
All Legal Guardians should be aware that alcoholic beverages will be available to youth members ages 21+.
Refunds will not be issued in the event that the waiver form is not completed & attendees are refused onboard
Valid ID proof is required to onboard Cruise. Drivers License is preferred, those who do not have a valid drivers license should bring a copy of a passport or any government issued ID that shows proper DOB.
After the process of application is completed, an email will be sent with a confirmation number to the Registrant with a waiver form.
Please bring signed waiver form (20 below ONLY) on the day of the event & hand to the check-in team
No Refunds / Exchanges / Cancellations are accepted for any type of Registrations.
The TTA organization & TTA Youth Team will not be responsible for members who decide to consume said beverages.
ID Bands will be handed to all members attending the cruise to differentiate the set age groups.
All attendees will be responsible for their transportation to and from the shuttle pick-up point at the Edison Sheraton Hotel.
Shuttles will depart promptly at 6:00pm to Hoboken Pier 14. Shuttles will return all attendees to Edison Sheraton after the cruise concludes at 11:00pm.
Please arrange proper transportation back to your homes/hotel rooms.
